Ordering Information

How to place an order at scanserv.com

1. Complete both pages of the order form and make sure all of the information is correct. Once you submit the second page, your order has been placed. Remember to include shipping information. If you have any questions, email customerservice@junglelasers.com or call 877.502.2789 (8am - 7pm EST).

2. You must print the order form on the "Thank you" page and include this orderform with your submission. If you are paying by credit card, include your VISA or MasterCard Account Number, Expiration Date, and Billing Address. DO NOT EMAIL CREDIT CARD INFORMATION TO SCANSERV.COM. If you are paying by check, please include your check number on the printed orderform. A scanserv.com representative will contact you to confirm your order and to find out when the documents will be shipped. Credit card payments can be completed at this time. If you are paying by check, please make checks payable to Jungle Lasers, LLC and mail with your documents. Include the check number on the order form.

3. All documents will be scanned at the requested DPI format. All files will be saved to a read-only CD-ROM. Click here to view the example images.

4. Mail completed order form along with documents to scanserv.com c/o Jungle Lasers, LLC, 201 Main St. Allenhurst, NJ 07711. Please include a completed return shipping address label. SCANSERV.COM IS NOT RESPONSIBLE FOR RETURN SHIPPING CHARGES. If no return shipping method is specified, we will be unable to process your documents.

5. Scanning Services will not be completed until full payment is received. VISA or MasterCard orders can be completed over the phone. You will not be charged until we receive your documents.
